While test sheets can now be filled in using a .PDF format and e-mailed to us for processing purposes, the originals must still be mailed in with completed pass/fail column, and instructor/examiner signatures. Certification cards will not be mailed until the original test sheets are received.
*Please Indicate whether you would prefer your certification card in French or English, when using bilingual test sheets.*
Save a blank copy of the test sheet to your computer, you can then fill it in and attach it to an e-mail.

Once the test sheet is filled out, use the "save a copy" button on the upper right of the document to save a copy to your computer. You can then attach it to an email.
Depending on which version of Adobe you are using the "save a copy" button may not allow you to save pre-entered information. Try filling in a few spaces and saving it to see if it works for you. If not then saving a blank copy is the option for you.
